Why Some People Aren't Suitable for LASIK

Some common reasons LASIK may not be recommended include:

  • Thin corneas
  • Irregular corneal shape (such as keratoconus)
  • High prescriptions beyond the safe treatment range
  • Significant dry eye disease
  • Certain eye or medical conditions (such as cataracts, glaucoma, retinal disease, or uncontrolled diabetes)
  • Lifestyle or occupational factors, particularly if there is a higher risk of eye trauma

 

The Good News: LASIK Isn't the Only Option

Being told you’re not suitable for LASIK doesn’t automatically mean you’ve reached the end of the road. In fact, many patients achieve excellent visual outcomes with other refractive procedures.

The best option depends on your prescription, eye health, age, lifestyle, and visual goals.

PRK (Photorefractive Keratectomy)

PRK is one of the original laser vision correction procedures and remains an excellent option today. Instead of creating a corneal flap, the surface layer of the cornea is gently removed before the laser reshapes the cornea. The surface naturally heals over several days.

PRK may be recommended for people with:

  • Thin corneas
  • Certain corneal characteristics that make LASIK less suitable
  • Occupations or sports where avoiding a corneal flap is advantageous

While recovery takes slightly longer than LASIK, the long-term visual outcomes are comparable.

SMILE (Small Incision Lenticule Extraction)

SMILE is another form of laser vision correction that treats short-sightedness and some levels of astigmatism through a very small incision rather than creating a large flap.

  • Potential benefits include:
    Smaller incision
  • Less disruption to corneal nerves
  • Reduced incidence of dry eye symptoms in some patients
  • Excellent visual outcomes for suitable prescriptions

Not everyone is a candidate for SMILE, but for the right patient it can be an excellent alternative.

Implantable Collamer Lens (ICL)

For patients with very high prescriptions or corneas that are not suitable for laser treatment, an Implantable Collamer Lens (ICL) may be recommended.

Rather than reshaping the cornea, a specially designed lens is placed inside the eye, behind the iris and in front of the natural lens. This means the natural lens remains in place, allowing patients to retain their natural ability to focus (accommodation).

ICL may be suitable for patients who:

  • Have high short-sightedness
  • Have thin corneas Prefer a procedure that preserves the natural cornea
  • Want a treatment that is reversible, as the ICL can be removed or exchanged

Because no corneal tissue is removed, ICL offers another pathway to reduced dependence on glasses for many patients who aren’t laser candidates.

Refractive Lens Exchange (Lens Replacement Surgery)

For some people, particularly those over 50 or those beginning to develop presbyopia (the need for reading glasses) or early cataracts, replacing the eye’s natural lens may provide the best long-term solution.

During refractive lens exchange, the natural lens is replaced with an artificial intraocular lens selected to suit the patient’s vision goals.

This procedure can:

  • Correct short-sightedness
  • Correct long-sightedness
  • Treat astigmatism (with toric lenses)
  • Reduce dependence on reading glasses (with selected lens designs)
  • Prevent future cataract development, as the natural lens has already been replaced

Why a Comprehensive Assessment Matters

The most important step isn’t deciding which procedure you want, it’s determining which procedure is safest and most appropriate for your eyes.

A comprehensive refractive assessment includes:

  • Corneal thickness and shape mapping

  • Measurement of pupil size
  • Review of your medical history, occupation, hobbies, and visual expectations

It’s natural to feel disappointed if you’re told you’re not suitable for LASIK. But modern refractive surgery is no longer about finding patients who fit one procedure. Instead, today’s approach is about matching the right treatment to the right patient.
